如何报名
Returning 和 new international students follow different enrollment processes. Please select the drop-down menu below to learn more about your specific enrollment process.
Fall 2022 semester Open-Enrollment has closed.
Watch your MyBGSU 'to do list' in October for the Spring 2023 semester's open enrollment period. All waivers 和 enrollments will be due on December 1, 2022.
Fall 2022 Open Enrollment is closed.
If you will be a new international student, you are required to complete the Digital Falcon 健康 Packet. The following documents are included in this packet:

- 健康检查表格
- 保险登记表格
完成后, you are required to schedule a virtual appointment to answer your questions, review completed forms 和 determine if any follow-up actions are needed. A representative will reach out to you to book this appointment, but you can also contact them directly with any questions before your meeting at studentins@bjqzgy.com.
* If you are unable to complete your health packet prior to arriving to campus, we will assist you during orientation.
国际学生 Insurance 资格要求
家属 | 学生 who are enrolled in the plan may also insure their dependents (spouse, domestic partner 和 children until the age of 26.) Eligible 家属 will need to enroll into 和 pay for the health plan directly through United 健康care Student 资源.
Optional Practical Training Program | 国际 students that are enrolled into an Optional Practical Training Program are eligible for BGSU student insurance coverage.

国际学生 Insurance Enrollment 豁免
The majority of international health insurance plans are not accepted in the United States 和/or Northwest Ohio, which is why you are not able to waive the plan on this web page.
You are required to enroll in the BGSU student health insurance plan by the semester deadline, unless you meet one of the following exemptions:
- 学生 with approved coverage that is provided by their government /embassy
- 学生 with approved coverage provided by a U.S. 基于雇主
- 学生 who have coverage through approved policies
If you believe you meet one of these exemptions, you will need to contact the Student Insurance Office between 8 a.m. 还有5p.m. 美国东部时间周一至周五 studentins@bjqzgy.com，用你的 健康保险卡, BGSU学生证 和 health insurance policy information 为了完成弃权书.
猎鹰健康中心
The 猎鹰健康中心 accepts the BGSU student insurance plan 和 most insurance policies 和 is located next to campus. The center serves the entire surrounding community as well as the BGSU faculty, staff 和 students.
猎鹰健康中心 offers convenience care appointments with medical providers, 免疫接种, 旅行医学, 皮肤检查, 实验室和放射服务, 针灸, chiropractic care 和 physical therapy.
For your convenience the 猎鹰健康中心 also offers a full-service pharmacy.
